You do not need a referral to see me for PIP/MVA-related treatment. You do need a Claim Authorization # from your insurance company.
If your health insurance plan covers Out-Of-Network Providers for Acupuncture: I may be able to bill your insurance plan.Some plans require members to pay a "co-pay" or "co-insurance" fee at the time of treatment. Co-insurance is a percentage of your billable visit cost and is determined by your insurance plan.
Super-bill: If your health plan policy covers acupuncture by out-of-network providers. You pay the full amount for the visit at the time of service. An itemized receipt is provided to you to submit to your health plan for reimbursement directly to you. Note: this does not guarantee that you will receive full reimbursement from your insurance company. Every plan is different.
