As of January 2023 I no longer bill insurance plans. Payment is due at the time of service.
I accept payment by cash, check, and most credit cards (Square-Up). A small processing fee will be added to the visit cost. You may also choose to use: Flex-Spending Account cards (FSA) Health-Savings Account cards (HSA)
If your plan covers Out-Of-Network Providers: I may be able to bill your insurance plan for visits.Some plans require members to pay a "co-pay" or "co-insurance" fee at the time of treatment. Co-insurance is a percentage of your billable visit cost and is determined by your insurance plan.
Super-bill: If your health plan policy covers acupuncture by out-of-network providers. You pay the full amount for the visit at the time of service. An itemized receipt is provided to you to submit to your health plan for reimbursement directly to you. Note: this does not guarantee that you will receive full reimbursement from your insurance company. Every plan is different.
Motor Vehicle Accident (MVA) Claims Personal Injury (PIP) Claims. You do not need a referral to see me for PIP/MVA-related treatment. You do need a Claim Authorization # from your insurance company. I am happy to work in collaboration with any healthcare professionals you are seeing.
